Physical Upload Information for Summer Workouts and Camps
Families should complete physicals now and keep a copy of Page 5 of the MSHSAA form. New physical uploads into PowerSchool reopen July 13.
What Families Need to Know Right Now
PowerSchool uploads are temporarily unavailable for new physicals until July 13.
Families should still complete the physical process now and keep a copy of Page 5 of the MSHSAA Physical Form.
This upload delay does not cancel or change scheduled physical days, workouts, camps, practices, or summer activities.
Log in to PowerSchool
Parents should log in to PowerSchool and go to the same area where student registration forms are completed.
Note: New physical uploads cannot be completed until July 13, but families may still complete the other registration steps as available.
Mark that your student will participate in activities
On the first page of the enrollment or registration form, make sure you select that your student will participate in activities.
This is important. If this is not marked correctly, the activity and athletic forms may not show up the way they need to.
Complete Form 5A-Act/Ath
Next, go to the tab or form labeled 5A-Act/Ath.
Parents and students will need to watch the required parent/student video and acknowledge that it has been completed.
Prepare the physical for upload
Once uploads reopen on July 13, the physical should be uploaded in the 5A-Act/Ath section.
Families may upload the physical as either:
- A clear photo
- A clear PDF
The file must be clear, fully visible, and in focus so the school can review it.
Only upload Page 5 of the MSHSAA form
Families only need to upload Page 5 of the MSHSAA physical form. This page is the MSHSAA Medical Eligibility Form.
Page 5 must be completed by the healthcare provider and kept by the family until PowerSchool uploads reopen on July 13.
Before uploading, check that Page 5 is complete
Before submitting the form in PowerSchool after July 13, please make sure Page 5 includes:
- The athlete’s information filled out
- At least one eligibility box checked by the healthcare provider
- The exam date
- The healthcare provider’s signature
- The provider’s office information
If any of these items are missing, the physical may not be accepted.
Wildcat Physicals Available June 9
- Date: June 9
- Time: 10:00 a.m. – 12:30 p.m.
- Location: Blue Springs High School — enter Door C16
- Cost: $30 cash only
The full fee from these physicals is returned to BSHS Football.
This event is still happening. The PowerSchool upload delay does not affect scheduled physical days.
Important Reminder
New physicals cannot currently be uploaded into PowerSchool until July 13.
Families should still complete physicals, keep a copy of Page 5, and be ready to upload once the system opens.
If your student had a two-year physical uploaded in Privit last year and you need a copy, please contact the athletic office at your school.
